Book an Appointment: 3 Easy Steps

1. Create Appointment Online (new patients, please complete step 3)
2. Verify your insurance by emailing the following information to susierobe@yahoo.com:
   • Insurance company name
   • Subscriber ID
   • Date of birth
   • Contact phone number on back of card
   • Diagnosis (example: diabetes, cholesterol, high blood pressure, etc.)
3. Complete our Online Assessment Form - (new patients please complete)
